Take Advantage of Remote Therapeutic Monitoring (RTM) CPT Codes with CarePaths Measurement-Based Care by Bruce Wampold PhD
This year, the American Medical Association added a number of new CPT® codes to its catalog that will allow mental health providers practicing Cognitive Behavioral Therapy to bill their patients’ insurance for Remote Therapeutic Monitoring services. These additions are expanding upon the existing set of RTM codes which focus on specific aspects of physical health. CarePaths users whose clients are receiving CBT and completing assessments in the CarePaths Connect mobile app for patients can now bill for time spent training their clients on using the app (CPT code 98975) as well as time spent completing and discussing the assessments (98980 for the first 20 minutes and 98981 for each additional 20 minutes during a month). Patients are automatically notified to complete these assessments on a weekly basis, allowing their providers to bill using code 98978 once per 30-day period as well. Talking to Patients as Part of Measurement-Based Care
As we have discussed previously, measurement-based care (MBC) involves assessing patient progress and the therapeutic process throughout the course of treatment to improve the quality of the service being delivered. Of course, the clinician will naturally gather many of their own insights from interacting with the patient. MBC supplements this information by providing the patient’s perspective on their symptoms, well-being, and their relationship with the clinician.

Administering Assessments: The Pragmatics of Measurement-Based Care (MBC)
Once a clinician or a system of care has made the commitment to implement MBC, we soon find that the devil is in the details. Like most initiatives, the pragmatics involved in MBC, particularly administering the assessment to patients and collecting responses, is critical to its success. The challenge is to collect useful and actionable information without burdening patients, clinicians or administrators. In this blog, I will discuss the major challenges to administering assessment to patients and touch upon the CarePaths solution. As with any systemic initiative, there are tradeoffs to consider, depending on the context and priorities.

An Introduction to CarePaths’ Approach to MBC
Measurement-based care (MBC) involves assessing patient progress and therapy processes through the course of therapy to improve the quality of the service being delivered. A number of different labels have been used for this practice, including routine-outcome monitoring (ROM), practice-based evidence, feedback informed treatment (FIT), among others. Whatever name is used, the research is clear: Providing information about patient progress to the therapist improves outcomes, especially for cases that are not on track to be successful. MBC is now considered an evidence-based practice and the American Psychological Association is developing practice guidelines for its implementation.

CarePaths Connect is Live!
We are excited to announce that CarePaths Connect, the new Digital Front Door for therapists and patients is now available. Current users of CarePaths EHR can upgrade to CarePaths Connect for free by having your practice admin sign into their account and select CarePaths Connect under the Admin tab. Once CarePaths Connect has been activated for the practice, every clinician will find a link on their homepage that says “Configure Digital Front Door.” After clicking this link, use your regular login information and you’ll be able to set up your profile as you’d like it to appear to users of the app. Then you can send an invitation to your current patients to download the app, available for iOS and Android. Your Digital Front Door will also be visible to new patients using the app to search for a therapist. You can list your open appointments so patients can conveniently request time with you. New patients will also be able to enter their insurance information in the app to find out which therapists accept their plan and what their copay will be. For more detailed instructions, see the release notes in the System Updates section of the Help Center in the CarePaths EHR as well as this series of tutorial videos.We will also be setting time aside to run live trainings on the new system with our users. You can find more information on how to register for these trainings in the release notes.

What do Outcomes Monitoring Systems really measure?
Behavioral health professional and regulatory organizations are stepping up support for routine outcomes monitoring (ROM) (also referred by by terms such as Outcomes Monitoring, Feedback Informed Treatment, Measurement-Based Care, etc). They encourage clinicians to monitor patients’ responses to treatment and identify those at risk of a bad outcome and those who are progressing as expected. ROM is also promoted to compare the effectiveness of clinicians. But a soon to be published analysis suggests ROM systems have overstated their case and recommends caution by clinicians using them.
